Please download a copy of the Certificate of Catholicity and Certificate of Catholic Practice (click on the link below), or take a paper copy from the blue tray on the table in the Church porch. Please complete both forms and place them in an envelope with your Child’s name on the front and place the envelope in the blue tray in the porch. Your form will be completed and returned to the blue tray.
