If you are concerned that the parish of Hemel West has not received your Certificate of Catholic Practice for Senior School 2024, please resubmit another form using the information below or call Shirley in the parish office on 01442 391 759 and select option 2 to check to see if she has received it.
Please ensure that your application for a Certificate of Catholic Practice is received at the Hemel West parish office by Friday 13 October. Any applications for CCP for Senior School 2024 will not be signed after this date, and it will your responsible for any late applications.
Certificates of Catholic Practice can be downloaded from the link here. Once you have completed the form it should be posted through the Hemel West parish office letterbox at 186 St Johns Road. If everything is fine with your application it will be signed and sent to the school for processing. The Parish Office is behind the Parish Centre, Boxmoor. Just follow the signs outside the Presbytery to the Parish Office. You’ll see the opening times below.
